My niece passed away 4 months ago and we just got her toxicology report back and it said she had a fatal dose of hydromorphone 0.13ml in her blood. What dose would she have had to taken to get a blood level like that?

First off, why was she taking it? If she was given a larger dose than necessary, it can cause slowed breathing/respiratory distress. Is it liquid or other form? More info would give me a way to answer this question thoroughly. Also if she had thyroid disease, adrenal problems, gallbladder disorders, or had a sulfite allergy, that would make the dose fatal. 

EDIT...Just found out this medication is not approved safe for anyone under 18!
