I would say that yes, Tylenol with codeine will act much the same as a Vicodin or Norco capsule.

Normally the combination of Tylenol with codeine is used to relive moderate to severe pain, as are Vicodin and Norco.

  • Tylenol with codeine, Norco and Vicodin are all painkilling opiates.
  • Tylenol contains acetaminophen - a slightly less-potent opiate which accelerates and heightens the effects of the codeine already in the mixture.
  • Vicodin also contains acetaminophen, although in this case it is mixed with hydrocodone (a semi-synthetic opiate derived from codeine and thebaine).
  • Norco, like Vicodin, is also a mixture of acetaminophen and hydrocodone.
  • In Tylenol the acetaminophen is mixed with the naturally-occurring substance codeine, but both Norco and Vicodin use the semi-synthetic hydrocodone as the main opiate.
  • They all have much the same painkilling effect in the body and all will cause the user to fail a drug screening. This is because of the presence of opiates in these medications.

The actual answer: Yes, the bottom-line therapeutic effects can be the same -- when taken in equivalent doses. It takes 60 mg. Codeine to equal 9 mg. Hydrocodone. You can convert from there.

There's minor differences though. Codeine is converted into morphine, by your body, generally 10%. Hydrocodone though is converterted into other things (not morphine). I believe a tiny fraction of hydrocodone is converted into hydromorphone, I'm not sure exactly the percentage.

So, for 'recreational purposes' the 'high' will be slightly different. Codeine gives a 'heavy' feeling (called a 'body buzz') + mild relaxation while hydrocodone just gives a 'light', happy/giddy feeling. I've also found that codeine is far more consistent. Hydrocodone works the first few times you take it, but after that it's like your body doesn't even recognize it anymore. Not the case with codeine, in my experience.

Codeine is slightly weaker, but yes, they will both get the job done. 15 mg codeine and 5 mg hydrocodone (Vicodin) will have the same painkilling effect, though unless you have no tolerance whatsoever those low doses will do nothing for a subjective high.
