Never and never to my primary care doctor. IMO, there's no reason to lie.

Like Matt said ... You are paying for their service. If you lie, it means you may not get the proper treatment - so you would just be wasting your own money.
What would I lie about? I brush and floss regularly and have nothing to hide lol
I did once. Lol it was so funny my braces bracket broke when I bit a lollypop and it was that way for a week. When they asked, did you know your bracket was broken? I was like, it is?
