sounds like this might be some sort of infection.Get him to try washing it out with hot water before rubbing anti-bacterial gel on it.
He also needs to keep it as dry as possible after that.
If it continues in the same way, or gets any worse, then he needs to see a doctor as infections may spread, and if tissue damage occurs, that tissue may not grow back.
