Before, I was 112 lbs (girl), now I am 354 lbs. I don't eat 100% healthy, but I don't completely junk food either. I soon went up to 150, then 200, 250, to 350. I don't want to weigh 400. How can I lose the weight and how did I get fat?

Overeating and lack of regular exercise. You do eat a lot of junk food and you need to be brutally honest with yourself. Seek counseling and nutrition assistance.
